Once you have completed the loan application, you must fax it or mail it to YOUR credit union office in Decatur or Galesburg, Illinois.  See the fax numbers and addresses below for the location closest to you.

IF YOU FAX THE LOAN APPLICATION, YOU MUST FORWARD THE ORIGINAL LOAN APPLICATION TO THE CREDIT UNION OFFICE.

The loan application may be reproduced as needed.  So for you Sub-Treasurers out there, feel free to make multiple copies to have on hand for members in your area.
